Applying for Determination of Eligibility for Children with Developmental Disabilities

In January 2013, the Department of Children and Families - Division of Children's System of Care (CSOC) assumed responsibility from the Division of Developmental Disabilities for determining eligibility of individuals under age 18 for developmental disability services. CSOC also assumed responsibility for providing support and services for those under age 21 with developmental disabilities.
The State of New Jersey has contracted with PerformCare to administer the publicly-funded developmental disability service delivery system for children up to age 21. Accordingly, applications for eligibility for developmental disability services for children under age 18 must be submitted to PerformCare. Examples of such services include Family Support Services which can be respite, camp, assistive technology devices, and home and vehicle modifications.*
If you are interested in applying for developmental disability services for your child, the application materials may be downloaded from www.performcarenj.org/families/integrated-services.aspx and printed out for completion by hand. You may also call PerformCare at (877) 652-7624 to request that the application materials be mailed to you. There is also a Frequently Asked Questions sheet on the webpage to provide additional information on both eligibility criteria and how to access services.
*It appears there are, at this time, no new funds available for Family Support. Families should check the website frequently for updated information, as well as information about all the services offered through the CSOC.
